Improve request form template functionality

It is great that you can create templates for request forms, but the functionality is pretty limited. I would like if there was a way to:

• Edit templates to add or remove fields. Updates to templates could optionally be pushed out to all services using them.

• Save conditional logic in association with templates. This is a big pain point that all conditional logic needs to be carried over manually.

• Generally the form editing experience is clunky and buggy.
