Add an option to remove the service name / image

It would be amazing to add an option to remove the service name / image on the page where "Create request" is :)

"Hello! I have another question, when a client create a new request, there is a grey box about the service they are putting a request in, I don't really understand why it is there, would it be possible to remove it?"
